The January 1938 Post Office Monthly Circular gave the following information about the issue of the Souvenir telegram form for the Sesqui-Centenary Celebrations in New South Wales:
"Approval has been given for the delivery of telegrams on souvenir telegram forms from certain offices in connection with the Sesqui-Centenary Celebrations in New South Wales during the period 26th January to 25th April 1938.
The Souvenir telegram may be lodged at any telegraph Office in the Commonwealth, but delivery on the special stationery shall be effected only from Post Offices in New South Wales, classified as Grades 2, 3, 4, 5 and 6, and from the Chief Telegraph Office in each of the other States.
Should a request be made to have delivery effected to an addressee residing in a district but served by a post office of the above mentioned grades or by a Chief Telegraph Office, it will be necessary for the word "post" to be inserted followed by the name of the office from which the telegram will be posted; or, if that name is not known at the office of origin, by the word "nearest".
The supplementary instruction should be counted and charged for as two words.

There is no information about how many forms were issued. Certainly there are very few used forms recorded and none known in a tied pair with the delivery envelope.
